Reading: 30 dakika, 21 soru
Reading bölümü gerçek sınavda tüm sağlık meslekleri için ortaktır. Süreyi başlat ve bitirince “Cevapları kontrol et” butonuna bas.
Part A: Anaphylaxis: emergency management sorular 1–10 · önerilen süre 8 dakika
Part A hızlı okuma bölümüdür: dört kısa metni tarayıp bilgiyi hızla bulman gerekir. Gerçek sınavda bu bölüm 15 dakika sonra toplanır.
ARecognising anaphylaxis
Anaphylaxis is likely when all of the following are present:
- sudden onset and rapid progression of symptoms;
- life-threatening airway and/or breathing and/or circulation problems;
- skin and/or mucosal changes such as flushing, urticaria or angio-oedema.
Skin changes on their own are not a sign of anaphylaxis, and in some patients they are subtle or absent altogether. Exposure to a known allergen supports the diagnosis but is not always identified.
BIntramuscular adrenaline (1 mg/mL, 1:1000)
Adult and child over 12 years: 500 micrograms (0.5 mL)
Child 6–12 years: 300 micrograms (0.3 mL)
Child 6 months–6 years: 150 micrograms (0.15 mL)
Inject into the anterolateral aspect of the middle third of the thigh. If there is no improvement, repeat the dose after 5 minutes. Do not delay adrenaline while waiting for intravenous access.
CObservation and investigations (hospital policy)
Observe all patients for a minimum of 6 hours after symptoms have resolved. Extend observation to at least 12 hours if more than one dose of adrenaline was required or if the patient has a history of biphasic reactions.
Take a blood sample for mast cell tryptase as soon as possible after emergency treatment has started. A second sample should be taken ideally within 1–2 hours, and no later than 4 hours, from the onset of symptoms.
DBefore discharge
- Prescribe two adrenaline auto-injectors and demonstrate the correct technique.
- Advise the patient to carry both auto-injectors at all times.
- Refer the patient to a specialist allergy service for further investigation.
- Provide written information on avoiding the suspected trigger. For children, ensure the school has a copy of the emergency plan.
8A patient who needed more than one dose of adrenaline should be observed for at least hours.
9The second tryptase sample should be taken no later than hours after symptoms began.
10Before discharge, the patient should be referred to a specialist for further investigation.
Part B: iş yeri metinleri sorular 11–13
Part B'de not, e-posta, kılavuz gibi kısa iş yeri metinleri var. Soru çoğunlukla metnin amacını ya da ana mesajını sorar.
Memo to all ward staff
From 1 March, hand hygiene audits on all wards will be carried out unannounced by members of the Infection Prevention team rather than by ward staff themselves. This change follows concerns that self-audits have consistently overestimated compliance. Results will be shared with ward managers within 48 hours, and wards scoring below 90% will receive additional training sessions.
Email: medication refrigerators
Please remember that refrigerator temperatures must be recorded every day, including weekends. If the reading falls outside the range of 2–8 °C, do not discard the medicines. Label them 'Do not use', keep them refrigerated and contact Pharmacy, who will advise whether they can still be used safely.
Guideline extract: verbal orders
Verbal or telephone medication orders should be accepted only in an emergency or when the prescriber is unable to attend in person. The nurse receiving the order must read it back to the prescriber to confirm accuracy, and the prescriber must countersign the order in the patient's record within 24 hours.
Part C: Antibiotics: is less really more? sorular 14–21
Part C'de uzun bir sağlık makalesi okursun. Sorular yazarın ve uzmanların görüşlerini, bir ifadenin neyi kastettiğini ve paragrafların amacını sorar.
Antibiotics: is less really more?
For decades, patients were told to finish their course of antibiotics, even if they felt better. The advice was intended to prevent resistance: stopping early, the reasoning went, would leave the hardiest bacteria alive to multiply. Yet a growing body of evidence suggests that this long-standing message may have been, at best, an oversimplification. For many common infections, shorter courses appear to be just as effective as longer ones, and they expose the body's bacteria to fewer days of selective pressure.
Dr Helen Marsh, a consultant microbiologist, argues that resistance develops chiefly in the bacteria that normally live harmlessly in the gut and on the skin, rather than in the organism causing the infection. "Every additional day of treatment gives these bystander bacteria another opportunity to adapt," she explains. "Prolonging therapy beyond what is needed doesn't protect the patient; it simply increases the chance that the next infection will be harder to treat." Trials in community-acquired pneumonia and uncomplicated urinary tract infections have shown that courses of three to five days often produce outcomes comparable to the traditional seven to ten.
Not everyone is ready to abandon the old rule, however. Some general practitioners worry that a change in messaging could cause confusion. Dr Raj Patel, who runs a busy urban practice, points out that patients rarely distinguish between 'stop when you feel better' and 'take a shorter course as prescribed'. "If people start deciding for themselves when to stop, we could see more treatment failures, particularly in older patients, whose symptoms can be subtle," he says. In his view, the solution lies not in telling patients to stop early but in prescribers choosing the shortest effective duration from the outset.
This position reflects the direction taken by many national guidelines, which now specify shorter default durations for common infections. Nevertheless, guidelines alone cannot change practice. Audits repeatedly show that a substantial proportion of prescriptions exceed recommended durations, often because clinicians default to familiar habits or prescribe 'just in case'. Antimicrobial stewardship teams have therefore begun to focus on the point of prescribing, building recommended durations into electronic systems so that a five-day course, for example, is the automatic option rather than an active choice.
Such measures appear to work. In one hospital trust, introducing default durations reduced the average length of antibiotic courses by nearly two days without any increase in readmissions. Yet stewardship experts caution against complacency. Shorter is not always better: deep-seated infections such as osteomyelitis or endocarditis still require prolonged treatment, and blanket rules risk undertreating these patients. "The goal isn't the shortest course for everyone," Dr Marsh stresses, "but the right course for each patient."
Ultimately, the debate highlights a broader challenge in communicating evidence to the public. Messages that are simple enough to be memorable are often too simple to be accurate. As our understanding of resistance evolves, clinicians will need to explain not only what patients should do, but why, if trust in medical advice is to be maintained.

Dear Dr Collins,
Re: Mr Daniel Hughes, aged 62
I am writing to refer Mr Hughes, a retired bus driver, for assessment of suspected stable angina.
Over the past three weeks, he has experienced chest tightness on exertion, such as walking uphill, which resolves with rest within five minutes. He has had no pain at rest.
His history includes hypertension and type 2 diabetes, and he is an ex-smoker with a 30-pack-year history. Of note, his father had a myocardial infarction at 58.
On examination, his blood pressure was 148/90 mmHg. His ECG showed sinus rhythm without acute changes, and his total cholesterol was raised at 6.4 mmol/L.
I have started aspirin, atorvastatin and a GTN spray, and advised him to call 999 if the pain occurs at rest or lasts longer than 15 minutes.
I would be grateful if you could assess him and arrange further investigations.
Yours sincerely,
Doctor

Setting: General practice
You are a GP. The patient, Mr/Ms Aydın, aged 54, was diagnosed with type 2 diabetes last week (HbA1c 58 mmol/mol). Diet changes alone have not been enough, and you have recommended metformin.
Task:
- Find out why the patient is reluctant to start metformin.
- Explain the diagnosis in simple terms and why treatment is important.
- Explain how metformin works and its common side effects (stomach upset, usually settles; take with food; dose increased gradually).
- Address the patient's concern about 'being on tablets for life'.
- Agree on a plan and arrange follow-up.
Hasta kartı (partnerin okusun)
You were diagnosed with diabetes last week. You are worried about side effects because a friend had stomach problems on metformin. You also feel that starting tablets means you have 'failed'. Ask whether you can try diet and exercise for longer.

Açılış
Hello, I'm Dr ___. I understand we're here to talk about the results from last week. How have you been feeling since then?
Endişeyi keşfetme
I sense you have some reservations about starting the tablets. Could you tell me what's on your mind?
Empati
It's completely understandable to feel that way, and many people do when they're first diagnosed.
Açıklama
In simple terms, your body isn't using sugar as well as it should, so the level in your blood stays too high. Over time, that can affect your eyes, kidneys and heart.
Yan etkiler
Some people do get an upset stomach at first, but it usually settles within a few weeks. Taking it with food and starting with a low dose makes a real difference.
Hastanın bakış açısı
Starting medication certainly doesn't mean you've failed. Your efforts with diet are still essential; the tablets simply work alongside them.
Uzlaşma
Would you be willing to try it for three months, and then we can review your results together?
Kapanış
If you notice severe stomach pain or vomiting, please contact us. Is there anything else you'd like to ask?
