Reading: 30 dakika, 21 soru
Reading bölümü gerçek sınavda tüm sağlık meslekleri için ortaktır. Süreyi başlat ve bitirince “Cevapları kontrol et” butonuna bas.
Part A: Pressure injury prevention sorular 1–10 · önerilen süre 8 dakika
Part A hızlı okuma bölümüdür: dört kısa metni tarayıp bilgiyi hızla bulman gerekir. Gerçek sınavda bu bölüm 15 dakika sonra toplanır.
ARisk assessment
All adult inpatients must have a pressure injury risk assessment completed within 6 hours of admission, using the Braden scale.
A score of 18 or below indicates that the patient is at risk. Reassess daily and whenever the patient's condition changes, for example after surgery or a period of sedation.
BRepositioning schedule
Braden 15–18 (mild risk): reposition every 4 hours
Braden 13–14 (moderate risk): reposition every 3 hours
Braden 12 or below (high risk): reposition every 2 hours
Use a 30-degree tilt and avoid positioning the patient directly on the hip bone. Keep the head of the bed at 30 degrees or lower unless this is contraindicated.
CEquipment
- At-risk patients: high-specification foam mattress.
- High-risk patients, or those with a category 3 or 4 injury: dynamic (alternating pressure) mattress.
- Use heel-offloading boots for patients who cannot move their legs independently.
- Do not use ring-shaped cushions ('donut rings'), as these concentrate pressure on the surrounding tissue.
DSkin inspection and reporting
Inspect the skin each time the patient is repositioned and record findings on the SSKIN care bundle chart.
Any pressure injury of category 2 or above must be reported as an incident within 24 hours. With the patient's consent, photograph the wound for the clinical record.
8A patient with a Braden score of 13 should be repositioned every hours.
9The skin should be inspected each time the patient is .
10A category 2 injury must be reported within hours.
Part B: iş yeri metinleri sorular 11–13
Part B'de not, e-posta, kılavuz gibi kısa iş yeri metinleri var. Soru çoğunlukla metnin amacını ya da ana mesajını sorar.
Memo: bed rails
Following a review of recent incidents, staff are reminded that bed rails should not be used routinely for patients with delirium or agitation. Confused patients may attempt to climb over the rails, resulting in falls from a greater height. A falls risk assessment must be completed before bed rails are used for any patient.
Notice: peripheral cannula care
Inspect every peripheral cannula site at least once per shift and record a Visual Infusion Phlebitis (VIP) score. If the score is 2 or above, remove the cannula and document the reason. Cannulas should not routinely remain in place for longer than 72 hours unless there is a clinical indication.
Email: changes to handover
From Monday, the day-to-night handover will take place at the patient's bedside rather than in the staff office. Patients will be invited to contribute where appropriate. Any sensitive information that should not be discussed in front of the patient or visitors must be shared separately, away from the bedside.
Part C: Twelve-hour shifts: efficiency or false economy? sorular 14–21
Part C'de uzun bir sağlık makalesi okursun. Sorular yazarın ve uzmanların görüşlerini, bir ifadenin neyi kastettiğini ve paragrafların amacını sorar.
Twelve-hour shifts: efficiency or false economy?
Over the past two decades, twelve-hour shifts have become the norm on many hospital wards. For managers, the appeal is obvious: fewer handovers per day, simpler rotas and, at least in theory, lower staffing costs. Many nurses, too, welcome the arrangement, which allows them to work three long days a week and enjoy more days off. It is perhaps unsurprising, then, that the model has spread with relatively little scrutiny.
Recent research, however, has begun to challenge the assumption that longer shifts are as safe and efficient as they seem. A large European study found that nurses working shifts of twelve hours or more were significantly more likely to report poor quality of care and to leave tasks undone than colleagues working eight hours or less. Professor Anna Lindqvist, a nursing workforce researcher, believes the explanation lies in cumulative fatigue. "Concentration does not fall off a cliff at hour eleven," she says. "It erodes gradually, and nurses often don't notice it happening until a near miss brings it home."
The financial argument is also less straightforward than it appears. Although twelve-hour rotas require fewer handovers, studies have linked them to higher rates of sickness absence and greater reliance on agency staff, both of which carry considerable costs. In one analysis, any savings achieved through reduced handover time were largely offset within a year by these hidden expenses.
Not all experts are convinced by the case against long shifts. Mark Ellison, a ward manager in a large teaching hospital, argues that much of the research relies on nurses' own perceptions rather than objective outcomes. "Self-reported fatigue matters, of course," he acknowledges, "but it isn't the same as harm to patients." He also points out that eight-hour rotas create an extra handover each day, which itself introduces risk, since information can be lost every time care passes from one team to the next.
What both sides agree on is that the length of a shift is only one factor among many. Staffing levels, the opportunity to take proper breaks and the number of consecutive long shifts appear to matter at least as much. A nurse who works three twelve-hour days on a well-staffed ward, with protected breaks, may be at lower risk than one working five eight-hour shifts on a ward that is chronically short of staff.
For this reason, some organisations are moving away from a single model towards greater flexibility, allowing nurses to choose a mix of shift lengths. Early evaluations suggest that this approach may improve retention, although it places greater demands on those responsible for rotas. Whether flexibility proves to be a sustainable solution or simply a new administrative burden remains to be seen.

Dear Ms Price,
Re: Mrs Margaret Ellis, aged 79
Mrs Ellis is being discharged into your care today following an elective right total hip replacement, and I would be grateful if you could visit her at home within 48 hours.
She lives alone, although her daughter visits daily. She mobilises independently over short distances with a walking frame, and a raised toilet seat has been fitted. She is, however, anxious about falling.
Her 12 cm wound is clean and dry. The dressing should be changed only if soiled, and the clips are due for removal on day 12.
She will continue subcutaneous enoxaparin daily for 28 days; her daughter has been taught to administer it and is competent. She also takes regular paracetamol, codeine as required and a laxative, as she has had some constipation.
Please check her wound, remove the clips, support the injections and monitor her pain and bowels.
Yours sincerely,
Ward Nurse
Bu mektup neden işe yarıyor?

Açılış
Hello, I'm ___, one of the nurses on the ward. I understand you're going home today. How are you feeling about that?
Endişeyi keşfetme
It sounds as though you have a few worries. Would you like to tell me what's concerning you most?
Empati
It's perfectly natural to feel nervous after an operation like this, especially when you live on your own.
Enjeksiyon
The injection helps prevent blood clots. The needle is very small, and if you'd rather not do it yourself, a community nurse or a family member can help.
Düşme riski
At night, it's a good idea to keep a light on and your frame within reach, and to get up slowly rather than all at once.
Güvenlik ağı
If your wound becomes red, hot or starts to leak, or your calf becomes painful or swollen, please contact us straight away.
İtirazı yönetme
I understand you'd feel safer here. Medically, you're ready to go home, and recovery is often quicker in familiar surroundings. Shall we look at what support we can arrange?
Kapanış
Just to make sure I've explained things clearly, could you tell me what you'll do if the wound looks red?
